Medasist International provides service in all assistance services together with it’s solution partner Inter Partner Assistance –IPA ( AXA Assistance ). Medasist Travel Assistance in Turkey provides intermediary service in assistance service organization performed by Inter Partner Assistance( AXA Assistance).

As the Applicant of Medasist International I agree and acknowledge that all informaion written on this form is true, correct and my own declaration and accept to purchase Medasist International according to the provisions and conditions mentioned below. In case of any false at the declarations I agree and acknowledge that I will never claim any right and compensation.

GENERAL CONDITIONS
A) DEFINITIONS
1-Person: It means the person and/or persons who gain right the assistance service sold by Medasist or its partners. 2-Assistance Event: It means the events, which have been gained by the person within the scope of the Medical Assistance (Emergency Aid) Services all around the in Turkey 3-MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYMedical Team: It means the medical team, which consists of physician and/or allied health personnel, serves in 7/24 hours continuously within the organizational structure of MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYor acts for and on behalf of it, in order to evaluate any kind of medical ambulance necessity in case of any medical assistance occurs, and gives decision what kind of medical ambulance service will be performed by negotiating the physician who is in charge. 4-Accident: It means death or physical injury/damage of any insured person because of any immediate and external effect of any accident. 5-Physical Damage/Injury: It means any kind of physical injuries that requires the insured person should be medically examined by any healthcare organization or physician. 6-Ilness: It means the immediate and unforeseen illnesses occur at the first time and, which is not known before the date of agreement. 7-Serious Medical Cause: It means the serious injuries that put one’s life in danger. 8-Emergency Situation: It means the situations, which can not be prevented by the person and requires professional aid externally 9-Assistance Demand: It means the demand within the scope of the services as expresses in Section II that is submitted by the person or any other person who can acts for an on behalf of the said one. B) GEOGRAPHICAL COVERAGE Medical Assistance Lifestyle Services, which are mentioned in Section II, are valid in Turkey. SERVICES

A) MEDICAL ASSISTANCE
1. Medical Knowledge, Consultancy and Orientation Support
Regarding any kind of health problem that the card holder can meet urgently or nonurgently knowledge is provided by MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYMedical Team. Names, addresses and telephone numbers of the Medical Doctors, Specialist Doctors, Dentists, Hospitals, Diagnosis Centers and Pharmacies in charge are advertised. Recommendations for necessary precautions that should be taken regarding the existed health problems are made however no diagnosis is mentioned and no medicine is advised. If urgent medical attention is necessary all organizations are provided by MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYMedical Team.

For all medical transfer services;
2. Medical Transport
If the card holder becomes sick immediately or injured, MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYOrganization Team performs all necessary organizations regarding the transportation of the person with an ambulance (land or air ambulance) to the closest and suitable medical center or from any medical center to the other and commits to pay the service expenditures regarding these issues.
Transport from one Hospital to the other Transportation from one medical center to the other medical center can be performed on condition that there is no necessary medical equipments or medical team at the medical center regarding the medical treatment that will be done to the sick person. In order to realize the transportation of the sick person, a medical report that has been prepared by the medical doctor should be presented to MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYCompetent Officer.
Transport to home / domicile Moreover, when the patient is discharged if it is deemed as necessary by MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYMedical Team and Medical Doctor in charge, transportation of the patient his/her domicile will be provided (organization service) by land ambulance. For transferring hospital which is near home or residence, Medasist Travel Assistance in Turkey is only responsible from organization.
MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYMedical Team and physician in charge will determine whether the health condition of the insured person is suitable for transportation as healthy person, or necessary additional organizations are necessary or not and to the suitable transportation vehicle (land ambulance, air ambulance, scheduled flight regarding the damage and illness.
* Using of Air Ambulance decision is possible with the joint decision of the MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYAssistance Doctor and Patient’s Doctor, if there is any circumstances vitally importance.
* For transfer guarantee with air ambulance, service charge in Turkey borderlines will be undertaken by Medasist Travel Assistance in Turkey. However, for Europe and Mediterranean countries, Medasist Travel Assistance in Turkey only undertakes the organization.
Owner of Medasist Travel Assistance in Turkey will not have the right to redemption of expenses paid directly by him/her without prior consent of Medasist Travel Assistance in Turkey. For land ambulance, an approval must be obtained from Medasist Travel Assistance in Turkey.

B) TRAVEL ASSISTANCE
1. Information before Travel
Advance Information is provided to the card owner about visa, passports, vaccination requests at the entry, customs and their rules, currencies and taxes, weather situation, Banks, Embassies, and Consulates.
2. Loss of the Travel Documents
Procedure that should be followed in order to find out or replacement of credit cards, id cards or travel documents in case of lost or stolen and transactions should be done at the local authorities and associations.
3. Replacement of the Personal belongings
Replacement and sending of the broken, stolen or lost eye glasses, prosthesis and similar from Turkey is covered by MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEY. Expenses of eye glasses, lens and prosthesis are covered by the card holder or on condition to bring them MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYIstanbul Bureau, eye glasses, lens and prosthesis are sent to the country where the card holder is and delivered as well. Shipment expenses up to 5 kg are paid by MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEY.

*For fulfillment of this guarantee, the address stated by Medasist travel Assistance in Turkey owner must be in outside of Turkey.
4. Unforeseen Return to the Country (If the domicile is damaged seriously)
If the domicile of the card holder is damaged because of fire, or internal flood and in case of urgent return to the country, MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYpays the airline ticket of the scheduled flight in economy class for the card holder (by using the existed air line ticket as well) and organize the flight.

*For fulfillment of this guarantee, the address stated by Medasist travel Assistance in Turkey owner must be in outside of Turkey.
5. Flight Ticket Purchase / Transportation
Domestic and International flight ticket on behalf of Medasist Travel Assistance in Turkey owner can be purchased providing that prior consent of charge/place/location is obtained. Delivery is accomplished upon consent related with costs of flight ticket delivery to the address to be notified by owner of Medasist Travel Assistance in Turkey.
6. Notification to Family Members / Business Partners in case of delay
During the travel of owner of Medasist Travel Assistance in Turkey, any possible delays to be occurred in flight hours are notified by Medasist Travel Assistance in Turkey to family members or business partners upon demand of owner of Medasist Travel Assistance in Turkey.
7. Organization of the most suitable Medical Center in case of illness
If the person calls MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYbecause of his/her illness and demands, necessary organizations at the most suitable medical center according to him/her, will be realized on behalf of the card holder.

Expenditures arising from reservation, booking, ticket purchasing will be charged from the credit card by taking approval from the person.

Consulting Services
Legal Consultancy

A.1 DEFINITION:
Upon demand of owner of Medasist Travel Assistance in Turkey related with a legal issue, during 24 hours 365 days, Medasist Travel Assistance in Turkey will enable owner of Medasist Travel Assistance in Turkey to meet with a legal counselor Professional in his field, and by this way will provide legal counseling service. In counseling service to be provided, no legal document will be prepared; performance/ fulfillment of legal procedures on behalf of owner of Medasist Travel Assistance in Turkey will not be covered.
FINANCIAL CONSULT
Product Owner is informed by MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYFinance Consultancy Team, regarding any kind of financial problem that he/she meets. Any kind of consultancy service is provided to land lords and holders regarding financial and tax issues about the problems met. Necessary information is given regarding the transactions and activities that should be done and regarding legislations and general process. (If it is a comprehensive issue, providing of written explanation is preferred.)

** It includes to provide only information and consultancy services regarding the Consultancy Services by the specialists of the subjects.
I- Price of the Service Package of Medasist International Assistance is given above.
II- This agreement form will be come into force when the payment is collected by Medasist International.
III- Agreement Holder (Buyer-User) acknowledges and agrees that he/she does not have any right for termination of this agreement within the using year.
IV- Applicant agrees the special conditions of the Medasist International and Inter Partner Assistance (MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEY), which provide Assistance Services.
V- Owner of Medasist Travel Assistance in Turkey will not have the right to redemption of expenses paid directly by him/her without prior consent of Medasist Travel Assistance in Turkey.y.
VI- Applicant acknowledges and commits that the term of agreement is 1 (one) year for the Medasist Assistance Services and valid in the borders of Republic of Turkey and the all responsibility regarding the examinations and treatments and all services that will be provided by other than Medasist belong to these associations.
VII- Medasist International Services is valid only for Turkish Citizens, who live abroad (where their domiciles are located in foreign countries) and for the tourists, who come to Republic of Turkey for touristic, commercial, health purposes for 1 (one) year without considering their entrance and entry and exit numbers.

GENERAL EXEMPTIONS
1. Owner of Medasist Travel Assistance in Turkey will not have the right to redemption of expenses paid directly by him/her without prior consent of Medasist Travel Assistance in Turkey.
2. Damages caused by the following causes are excluded:
a) War, occupation, foreign enemy operation, conflict (whether the war has been declared or not), Civil War, rebellion, terrorist acts, military power, civil commotion.
b) Intentional self-injury or participation of an insured crime
c) damages which are occurred when the person has entered into any kind of car racing or demonstrations;
d) damages, which are arising from when the person has participated into any kind of fighting, quarrel, except from self-defense
e) damages, which are arising from when the person has participated into any kind of professional or demonstrative sportive events as being a competitive of demonstrator.
f) damages, which are arising from any kind of ionization radiations or radioactive contamination caused by any type of nuclear fuel or directly or indirectly any kind of nuclear component or radioactive and poisonous effects of the dangerous features of the nuclear components.
g) damages, which are arising from the under effect of alcohol intoxication (except or the drinking alcohol based on the suggestion of a physician as a part of medical treatment) or drugs or depend on the total or partially disability.
h) Death, injury or illness due to suicide or suicide attempt.
i) vents caused by spontaneous abortion intentionally until 3 months before the estimated date of birth
j) vents which are caused damage against the third persons or environments due to mental disability or depersonalization

OBLIGATIONS OF THE MEDASIST INTERNATIONAL OWNER
A. GENERAL PROCEDURE:
In Case any emergency situations, Holder of Medasist International Assistance calls MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYIstanbul Center by dialing the number 0216 ..................... (After you fill out your form and we received your payment via credit card, your service call center numbers will be send to you together with your certificate by e-mail) In 24 hours which was provided in advance without making any personal attempt.
 And informs his/her Surname and surname
 Contact telephone and place in order to be get in touch with him/her
 Identifies the problem that she/he met and type of the assistance demanded
B. MEDICAL TRANSPORTATION
If a person applies for Medical Transportation following issues should be provided:
i) For immediate reaction of MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEY
 Name of the hospital where he/she or supported person has been hospitalized
 Name of the physician in charge for treatment and name and phone number of the family doctor if any
ii) MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYMedcial Team or representatives should be able to access to holder of Medical International Assistance, in order to determine his/her situation. If performing of this obligation is not allowed than insured person except for having a reasonable objection will not gain any right for medical assistance.
iii) In any case MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYwill agree with the physician in charge for treatment mutually and will determine the date of transportation and type of transportation vehicle.
iv) In case of any hospitalization due to physical injury or damage, person or proxy who acts for and on behalf of the insured person will inform MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYat not later than 48 hours. If this information is not given within the specified period, MEDASİST INTERNATIONAL TRAVEL ASSISTANCE TURKEYwill gain right not to pay the expenses that can occurred from this hospitalization.
GENERAL SENTENCES Limitation
Medasist Travel Assistance in Turkey starts after 18 hours upon withdrawal of service charge from credit card of interested party upon approval of sales agreement after completing services forms
